Your account number is usually the last 4 digits of your call forwarding number. Your account number is also listed at the top of your daily or weekly log

This form is not to be used to indicate on-call coverage or changes to protocol.
Please be aware that if your oncall schedule runs after hours or on weekends only, to notify CMS of the oncall coverage for the Holiday.

If you need to add additional dates for when your office will be closed, please fill out the below form with your account number, company name and contact information. Your account number is usually the last 4 digits of your call forwarding number or the number listed at the top of your daily log you receive.

Using the applicable fields, please enter any additional holiday early leave and/or holiday closure hours by entering the date closing and date opening, along with the start and end times that the office will be closed.
Then click the blue "Save Date Range" button
